Dermal fillers and the Aging Process

As we age, we lose bone, collagen, and fat. Bone loss leads to diseases such as osteoporosis, but also to the hollowing of our face which makes us look older. Collagen loss leads us to have more dull appearing skin and more laxity, which can create fine lines and wrinkles.  As we mature, the loss of collagen and bone lets gravity pull on our natural fat pads in our face (that originally give us that youthful, inverted triangle appearance), and combine that with fat loss, gravity takes over, and our faces fall outwards and down, creating a more square faced appearance.

Hyaluronic Acid is a normal substance found throughout the body. It is a hydrophilic substance, and therefore, loves water. When we are dehydrated, our skin appears sullen and dark, but when we are hydrated, our skin appears plump and glowing. 

Topical or epidermal hyaluronic acid (HA) can help to maintain elasticity in the skin. HA dermal fillers will help fill lost volume.

Hyaluronic Acid Dermal Fillers

Hyaluronic acid fillers are just what they sound like: a substance that can be injected under the skin to help plump the skin and fill some finer lines and wrinkles by adding that substance which attracts water to the area.
There are many different types of hyaluronic acid fillers. Some are made to be strong and rigid so they can be placed in bony areas such as cheeks and jawline. Others are softer and more moldable to be placed in lips and fine lines.
Hyaluronic acid dermal fillers are completely reversible.

 

What to expect

Expect to arrive with a clean face to your appointment. Appropriate numbing medicine can be given to help with the process although most people just feel a small prick when the needle enters the skin. You will get appropriate counseling on the risks and benefits of getting dermal fillers, pictures taken of you in different angles both before and after the procedure. There could be some swelling and bruising immediately after the procedure. Cold compresses and Arnica Montana gel or pills will help to reduce the signs of swelling and bruising.
Hyaluronic acid dermal fillers will have an immediate result. You should leave the appointment happy with how you look. I never overfill anyone, as you can come back in two weeks for a touch up.
Pictures will be taken afterwards so that you can visually see what has been done by looking at all angles of your face.
A two week follow up appointment is a great idea to discuss further care, plan future treatments and if needed a touch up with any remaining areas that need it.

Am I a candidate for HA dermal fillers?

Anyone adult interested could be a candidate for dermal fillers. If you’re looking for an instantaneous lift or glow, ask your provider for what they recommend.

How long do HA dermal fillers last?

They can last anywhere between 6-18 months depending on the location placed, the type of hyaluronic acid and your own metabolism. Typically, lips tend to be treated every 6-9 months while cheeks tend to be treated every year to one and a half years.

Dermal Filler Pre-Care

Before getting dermal fillers, it is important to discuss your medical history with the physician, discuss any medications or supplements you are taking, as well as discuss any recent illnesses or vaccines you have had. I may ask you to stop certain medications or wait if you have had any illnesses or vaccines in the last 2 weeks.

Dermal Filler Post-Care

You will be able to drive after the procedure.
After receiving dermal fillers, you might experience swelling, bruising, and soreness in the area injected. Immediately using ice packs will ease the discomfort and swelling.
I recommend no vigorous activity for 2 days after treatment, and no airplane travel for 2 weeks after treatment.

Semi-Permanent Fillers also known as Biostimulatory Fillers

Semi-permanent fillers are those that actually stimulate collagen and bone to help restore the natural structure of your face and body. The two types of semi-permanent fillers that I use are Radiesse and Sculptra. Each can be used in different areas of the body to help restore bone loss in certain areas of the face as well as placing it more superficially to stimulate collagen formation to bring back that youthful glow and erase fine lines and wrinkles.

 

Radiesse

Radiesse is a semi-permanent type of dermal filler. It works on the architecture of your bone and collagen to maintain your natural bony architecture. It is also known as Calcium Hydroxyapatite.  This type of filler is an amazing natural looking filler that helps to stimulate bone and collagen formation. Radiesse can be placed directly on the bone to help build bone, and maintain the youthful appearance of the natural bony structures of the face.  

As this is a dermal filler that stimulates bone, the longevity of it is longer than that of hyaluronic acid fillers. The effects of Radiesse can last up to 2 years.  

Since Radiesse stimulates bone, it is also known to stimulate collagen formation by motivating sleeping fibroblasts to come alive and create new collagen. Therefore, by diluting the Radiesse with water or saline, we can use this just under the skin to help increase the collagen synthesis and this helps reduce the appearance of crepe-like skin, tighten some of the more lax areas that make us look and feel old.

How does Sculptra work?

PLLA stimulates fibroblasts. Fibroblasts are cells that cause collagen production. So Sculptra works on the initial phases of collagen formation. Over time, your body breaks down PLLA to water and carbon dioxide. The effects of PLLA appear gradually over a few months, producing natural results.

PLLA, is placed either in the deep layer, next to bone to stimulate fibroblasts to grow and build up and maintain your normal bony structure, or it can be placed more superficially, in the correct layer of skin, to stimulate the fibroblasts to make more collagen, which will leads to a tighter, more radiant façade.

 

What to Expect

Sculptra is not an instantaneous outcome. It has taken years for your skin and bones to age, and to reverse that, we need to take time to let the PLLA do it’s work. Each vial of Sculptra can create 2 ml of collagen. Typically, you will start with either 2 or 3 vials of Sculptra in your first appointment. Over the next 5 weeks, the PLLA does it’s job and over time you will start to see improvements in your skin quality. You will need to come back for a second and possibly a third appointment at 6 week intervals to get the desired effect.

The Sculptra is diluted in water in order to be placed in the exact locations to stimulate your fibroblasts. Feeling bruised or swollen for the first few days is normal.

 

What is the typical cost for dermal fillers?

Dermal fillers can cost between $700 to $1250 per syringe or vial. The reason there is such a difference in price is due to the type of filler as well as the advanced experience in injecting the filler in the correct location. Semi-permanent fillers cannot be reversed, so a high level of experience in injecting and a vast knowledge of anatomy must be known to prevent any possible complications. If a complication does arise, your injector should know exactly how to manage it so you can feel comfortable you are in good hands. You are paying not only for the product itself, but for the expertise of the physician injector and a tailored one on one experience making sure you feel and look your best.
